Applying for a green card or adjusting your immigration status requires a comprehensive medical examination by a USCIS-designated civil surgeon. We provide thorough, efficient immigration physicals with compassionate care during this important step in your immigration journey.
What is a USCIS Immigration Physical?
A USCIS immigration physical, also called a green card medical exam, is a required medical examination for individuals applying for lawful permanent resident status (green card) or certain other immigration benefits. The examination must be performed by a civil surgeon designated by U.S. Citizenship and Immigration Services (USCIS) and documented on Form I-693, Report of Medical Examination and Vaccination Record.
Who Needs an Immigration Physical?
You need an immigration physical if you are applying for adjustment of status to permanent resident (green card) while in the United States, applying for certain immigration waivers or relief, seeking asylum or refugee status (in some cases), or required by USCIS to complete Form I-693 for your immigration application.
The immigration medical exam is typically required for adjustment of status applications filed with USCIS, not for consular processing abroad (which has different medical exam requirements).
What to Expect During Your Immigration Physical
Your USCIS immigration examination at Broadway Family Clinic follows strict federal guidelines and includes:
Review of Medical History
We'll review your complete medical history including past illnesses and treatments, chronic medical conditions, previous surgeries or hospitalizations, mental health history, current medications, and substance use history. You'll need to provide documentation of any significant medical conditions.
Physical Examination
A comprehensive physical examination evaluates your general appearance and development, eyes, ears, nose, and throat, cardiovascular system, lungs and respiratory system, abdomen, lymph nodes, skin, and extremities and musculoskeletal system. The examination screens for any physical or mental disorders that would make you inadmissible under immigration law.
Vaccination Review and Administration
USCIS requires proof of certain vaccinations. We review your vaccination records and administer any missing required vaccines. Required vaccinations typically include Mumps, Measles, Rubella (MMR), Polio, Tetanus and Diphtheria Toxoids (Td or Tdap), Pertussis, Haemophilus influenzae type b (Hib), Hepatitis A, Hepatitis B, Varicella (Chickenpox), Influenza (Seasonal flu), Pneumococcal disease, Rotavirus (for children), Meningococcal disease, and COVID-19.
Age-appropriate vaccines are required. Some vaccines may be waived if medically contraindicated, and we'll provide appropriate documentation.
Tuberculosis (TB) Testing
All applicants aged 2 years and older must be tested for tuberculosis. We perform tuberculin skin test (TST) or blood test (IGRA) as appropriate. If your TB test is positive, you'll need a chest X-ray and possible evaluation for active TB disease. Our in-house X-ray services make this process more convenient.
Blood Tests for Certain Applicants
Applicants aged 15 years and older require testing for syphilis. If you have a history suggesting risk, additional STD testing may be recommended. Our in-house laboratory performs these tests efficiently.
Completion of Form I-693
After your examination and all required testing, we complete Form I-693 and place it in a sealed envelope. The sealed envelope must remain unopened and be submitted to USCIS with your adjustment of status application.
Required Vaccinations for Immigration
USCIS follows CDC vaccination guidelines. Required vaccines vary by age:
For All Ages (Age-Appropriate):
Mumps, Measles, Rubella, Polio, Tetanus and Diphtheria, Pertussis, Hepatitis A, Hepatitis B, Haemophilus influenzae type b, Varicella, Pneumococcal, Meningococcal, and Influenza (seasonal).
Additional for Children:
Rotavirus vaccine (for infants).
COVID-19:
COVID-19 vaccination is currently required for immigration medical examinations.
You must provide documentation of previous vaccinations. We can review your records and administer any missing vaccines during your appointment. Some vaccines require multiple doses over weeks or months, so plan accordingly.

The Immigration Physical Process
Step 1: Schedule Your Appointment
Contact our office to schedule your immigration physical. We'll explain what to bring and answer any questions about the process.
Step 2: Initial Examination
On your appointment day, we'll conduct the physical examination, review your vaccination history, and perform or order necessary tests (TB test, blood work if required).
Step 3: TB Testing Follow-Up
If you receive a tuberculin skin test (TST), you must return 48-72 hours later to have it read. If positive, a chest X-ray will be required (available in our clinic).
Step 4: Vaccination Administration
We'll administer any required vaccines you're missing. Some vaccines require multiple doses, which may necessitate follow-up visits.
Step 5: Form I-693 Completion
Once all examinations and testing are complete, we complete and sign Form I-693, place it in a sealed envelope, and provide it to you to submit with your USCIS application.
The entire process typically takes 1-2 weeks depending on TB testing results and vaccination requirements.
Inadmissible Medical Conditions
Immigration law identifies certain health conditions that may make applicants inadmissible. These include communicable diseases of public health significance (such as untreated active tuberculosis, untreated syphilis, gonorrhea, Hansen's disease), physical or mental disorders with associated harmful behavior, and drug abuse or addiction.
Most medical conditions do not affect immigration eligibility. If an inadmissible condition is identified, we'll discuss your options, which may include treatment and reevaluation or applying for a waiver.
Form I-693 Validity and Timing
Form I-693 is valid for 2 years from the date the civil surgeon signs it, or until you obtain permanent resident status, whichever comes first. The form must be signed no more than 60 days before you file your adjustment of status application with USCIS.
You can complete your medical examination before or after filing your I-485 adjustment of status application. However, strategic timing is important to ensure the form remains valid throughout USCIS processing.

Common Questions About Immigration Physicals
Many applicants have questions about the medical exam process. We're here to answer your questions, explain requirements clearly, guide you through each step, and help ensure your Form I-693 is completed correctly and accepted by USCIS.
After Your Immigration Physical
Once we complete your Form I-693, you'll receive a sealed envelope that you must submit to USCIS with your adjustment of status application. Do not open the sealed envelope—USCIS will reject opened forms. Keep the envelope in a safe place until you submit your application. If filing online, you may need to mail the sealed medical exam separately. Follow USCIS instructions carefully.
If USCIS identifies any issues with your medical exam, they may issue a Request for Evidence (RFE). Contact us immediately if you receive an RFE related to your medical examination, and we'll help address any concerns.

-
Bring any vaccination records you have. We'll review your immunization history and only administer vaccines you're missing or need boosters for.
-
Fasting is not required for standard immigration physicals. However, if you have a medical condition requiring additional blood tests, we may recommend fasting. Ask when you schedule your appointment.
-
positive TB test doesn't necessarily mean you have active tuberculosis disease. If your test is positive, we'll perform a chest X-ray (available in our clinic). Most positive TB tests represent latent TB infection, which is not contagious and doesn't prevent immigration. If active TB is suspected, you'll need treatment before USCIS will approve your application.
-
Most medical conditions do not affect immigration eligibility. Be honest about your medical history during the examination. We'll evaluate whether any conditions meet inadmissibility criteria and advise you on any necessary steps.
-
Some vaccines are safe during pregnancy, while others are not recommended. We'll review which vaccines you can safely receive and may defer certain vaccinations until after delivery if medically appropriate and permitted by USCIS guidelines.
-
Yes, children applying for adjustment of status require immigration physicals. Age-appropriate vaccinations and testing are required. We provide gentle, compassionate care for pediatric immigration exams.
